We want you to submit your best possible application to the MSQM: Health Analytics program—the one that most fully conveys your background, qualifications, and ambitions. We appreciate your time and care in making sure your application is complete, as the Admissions Committee can only review completed application packages. If you have questions after looking over the information here, please contact an admissions counselor at msqmhealthanalytics@fuqua.duke.edu.
To help decide whether this program is right for you, we encourage you to assess your personal situation and goals, consider the specifics of the program, attend an admissions event, and talk to family, friends, and mentors.
Application Requirements
Detailed application instructions are available in the online system. You’ll need to create a user ID and password to access the system:
- You may access the application as many times as you like before submitting it
- Your application will not be evaluated until you submit it and all supporting materials are received and verified
- You may only apply to one admissions round in a given application year
- You can view your application status by logging into your student portal
Here's what you'll need to complete your application:
Two essay questions and one short answer question must be completed before submitting your application. Prepare your responses carefully, as the Admissions Committee considers your answers important in the selection process.
Applicants have the opportunity to submit optional information explaining any extenuating circumstances about which the Admissions Committee should be aware.
All essays are scanned using plagarism detection software. Expressing your ideas by using verbiage from existing sources, including other applicants' essays or materials, without properly crediting those sources constitutes an act of plagarism. Plagarism is considered a cheating violation within the Fuqua Honor Code and will result in an application denial. Note: If you have worked with a consultant to complete your application materials, please ensure that the honor code policy is discussed and he/she will not share your essays with other potential applicants.
Essay 1:
Please describe your short-term and long-term career goals. How do you think Fuqua's MSQM: HA will help you achieve those career goals?
Your response should be about 1,000 to 1,500 characters, the equivalent of 200 - 300 words. Please respond fully and concisely using 1.5 line spacing. Do not copy the essay question in the document you upload with your application.
Essay 2:
Fuqua's MSQM program, though delivered online, is a cohort and team-based program. Your contributions to your classmates will help define your success in the program. How do you plan to draw on your past education and professional experiences to succeed in and contribute to the MSQM cohort?
Your response should be about 1,000 to 1,500 characters, the equivalent of 200 - 300 words. Please respond fully and concisely using 1.5 line spacing. Do not copy the essay question in the document you upload with your application.
Short Answer Question:
The Admissions team wants to get to know YOU--beyond your professional and academic achievements shared with us in the application. Please share with us ten random things about you. This could include important life experiences, hobbies, achievements, fun facts, or anything that helps us understand what makes you uniquely you.
Please present your response in list form, numbered 1 to 10. Some points may be brief, while others may be longer.
Legible, scanned copies of all unofficial transcript(s) are required for each college or university where you:
- earned (or will earn) a degree PRIOR to the start of the program to which you are applying;
- attended classes that relate to your graduate study interests.
High School Credits - if you took classes at a college or university while in high school, do not list the college or university as a separate school in your application.
Transfer and Study Abroad Credits - if the grades and course names appear on the transcript from your degree-granting university, you do not need to submit a separate transcript. If the grades and course names do not appear, please upload a clear, scanned copy of your unofficial transcript(s) from every college or university where you took a class related to your intended graduate field of study. You must upload a transcript from the original institution showing the courses and grades. If any courses were graded as pass/fail, you still need documentation from the institution confirming this. If you cannot contact the study abroad school directly, try requesting the documentation from your home university—they may have records from when the credits were transferred.
Transcripts must include:
- Your name and the name of the institution
- Each course taken and the grade received
- The degree received and date conferred
If your transcripts do not state the degree and date conferred, upload a copy of your diploma in addition to the transcript. If your transcript is not in English, it must be translated, attested, and uploaded along with the originals. Those applying from outside the U.S. should see additional transcript guidelines for international students.

If you do not submit your official transcripts on time, or if you misrepresent, falsify, or omit information, we may rescind any offer of admission or financial assistance.
When to submit copies of official transcripts
You will only be required to submit official transcripts for verification from each college or university you have attended if you are admitted to the program. The due date for official transcripts will be noted in your admitted student portal. If you have not yet completed your studies by this date, your official transcripts should reach The Fuqua School of Business no later than one week before your program begins.
Submit your required resume electronically within the online application system. For employment and volunteer positions, include the location, title, date, and responsibilities, starting with your most recent position. The education section should include dates of attendance and degrees received.
Your resume should include:
- Full-time employment
- Volunteer work
- Part-time work experience
- Education
One letter of recommendation is required for the MSQM: Health Analytics program. Your recommender must complete the current recommendation forms associated with the online application—no other format of recommendation will be accepted.
Please choose one individual who can objectively evaluate your professional skills and abilities. Recommendations from relatives and friends are strongly discouraged. Academic recommendations often provide a similar perspective to your transcript and are less helpful.
Submissions by email or mail are not accepted.
Other recommendation guidelines:
- You are required to use the recommendation forms that are within the online application.
- Use your recommenders' work email addresses, rather than their personal accounts, like Gmail, Hotmail, Yahoo, and so on.
- Recommendations from relatives and friends are strongly discouraged.
- Academic recommendations often provide a similar perspective to your transcript and are less helpful.
- Proprietors or principals of a company may provide a recommendation from a client, board member, or other professional colleague.
The following questions are included on the recommendation form:
- Describe the peer group to which you are comparing the applicant.
- Comment on the applicant's interpersonal skills (effectiveness in establishing and maintaining relationships; working with peers, supervisors, and subordinates; willingness to work in a team environment).
- What are the applicant's three primary talents or strengths? Please provide examples, if relevant.
- In what areas does the applicant need improvement or growth? If you have had the opportunity to provide the applicant with constructive feedback, please provide specific examples and the applicant's response to it.
- Please include additional comments you feel will be helpful to the Admissions Committee.
Bonus recommendation from Fuqua students and alumni:
In addition to the required recommendation, you may provide additional endorsements from our current students or alumni.
If one of these recommendations is sent on your behalf before you submit your online application, you will be eligible for an application fee waiver.
Standardized test scores are not required to apply to Fuqua’s Online MSQM programs. Applicants may choose to submit GMAT, GRE, or Executive Assessment (EA) scores if they believe doing so will strengthen their application. All tests are equally valued, and there is no minimum score requirement.
Applicants with a wide range of academic and professional backgrounds are admitted, and submitting a test score is entirely optional. The Admissions Committee evaluates each applicant’s quantitative readiness through academic performance, professional experience, and other indicators. In some cases, the Committee may request that an applicant take an exam to provide additional quantitative evidence before making a final decision.
If you choose to submit a test score:
- Report unofficial GMAT, GRE, or EA scores (from the same test date) in your application — practice tests are not accepted.
- If you’ve taken more than one exam format (for example, both the GMAT and GRE), you may submit your best score from each test type.
- Scores are valid for five years from the test date.
- If admitted, official test reports must be sent directly from the test administrator to The Fuqua School of Business.

Fee
A $125 USD non-refundable application fee, payable by VISA or MasterCard, is due when you submit your online application.
Possible application fee waivers and discounts
You may be eligible to receive an application fee discount ($75 USD off of $125 USD) if you meet at least one of the following criteria:
- Before submitting your application, you register for and attend at least one of the event activities listed below. Indicate the event type, location and month of attendance in your online application. (MBA Fairs are only considered as a qualifying event if you completed a sign in at the Fuqua table during the fair.)
- Campus visit
- Open house
- Information session
- Informational luncheon
- Meet & greet
- Multi-school information session
- Virtual information session
- You are an alumnus/alumna of Duke University. In your application, include the name of your degree and the year in which you graduated.
- You obtain an endorsement from a current Fuqua student or alumnus/alumna. In your application, include your recommender’s name, program (Daytime, Cross Continent, Weekend Executive, Global Executive, Master of Management Studies: Foundations of Business, Master of Management Studies: Duke Kunshan University, Master of Quantitative Management: Business Analytics, or Master of Quantitative Management: Health Analytics), year of graduation (if known), and email address.
You may be eligible for a full application fee waiver if you meet at least one of the following criteria:
- You are a member of the military—active duty, active reserve, or honorably discharged/completed service (within three years) of any nation. Either voluntary or compulsory service is valid for qualification.
- You are an active participant or alumnus/alumna of the Peace Corps, AmeriCorps, or Teach for America.
- You are a member of one or more qualifying organizations: Admit.Me Access, Forte MBA Launch, Fulbright, Management Leadership for Tomorrow (MLT), MBA Jumpstart, Posse Foundation, Reaching Out MBA (ROMBA), or Riordan Fellows Program.
- You attended one of our on-campus multi-day events:
- Duke MBA Workshop
- Duke MBA Women's Leadership Weekend
- Duke MBA Pride Weekend
- Duke MBA Veterans Symposium
Instructions to apply for an application fee discount or waiver
Meet at least one of the above criteria.
- Select an application round in the “Personal Information” section of the online application.
- Complete the questions included in the "Application fee, fee discounts, and fee waivers" section.
- Submit your application after all sections are complete. However, do NOT submit your application if you are asked for credit card information.
- Following the above instructions is the only way to receive an application fee discount or waiver. If you submit your online application without selecting the "fee waiver" or "fee discount" option, then you will not qualify for the waiver or discount, and no refund will be available.
An official interview is required, and your application isn’t complete without one. Once you have started an application, schedule your interview using our online interview system.
Interviewers will have a copy of your resume, but no other component of your application.
Interview fast facts:
- You must have started an application to able to access the interview scheduling tool.
- You do not need to have submitted/completed your application to schedule an interview
- Slots fill quickly, so schedule one at your earliest convenience
- Interviews are either in person at the Fuqua campus in Durham, NC, by phone, or by video-conference
- Interviews are conducted by an admissions officer, are conducted in English, and typically last 45 minutes to an hour
- If applying in the last round, your interview must be complete before the final admissions deadline
- Your interviewer will ask you a variety of questions about your educational background, work experience, and personal interests
- At the conclusion, you’ll have the opportunity to ask questions
In service to reapplicants, we offer the ability to replicate or 'clone' applications for those who submitted an application to the same program in the last two application cycles (2023-2024 or 2024-2025). If you are reapplying, please submit the Reapplicant Request form (login credentials required) to request that your previous application data be transferred over to the next available round of admission in the current application cycle. Please contact application-processing@fuqua.duke.edu should you have any questions or trouble logging in. You will receive a confirmation email once this process is complete, and can edit all or part of your application after it has been cloned.
The MSQM: Health Analytics program is open to international students. All students will need to be present on the Duke University campus for the mandatory program Orientation and team-building activities on the dates indicated on the academic calendar. Since there are no academic classes during that period, no F-1 student visa is required. It is the sole responsibility of every international student to arrange for their own entry to the United States.
Transcripts and diplomas
Transcripts, diplomas, and academic records must be in English and certified by an appropriate university official.
Transcripts must include:
- Your name and the name of the institution
- Each course taken and the grade received
- The degree received and date conferred
If your transcripts do not state the degree and date conferred, upload a copy of your diploma in addition to the transcript.
If your transcript is not in English, you must have it:
- Translated
- Attested
- Uploaded in addition to the originals

English as a Second Language test scores
Fuqua does not accept ESL test scores. We will use other aspects of your application to assess language proficiency.
Admission Decisions
You will be notified via email 2–4 weeks after you submit your application.
To reserve your place in the class, you must submit the following by the deadline specified in your admission notification:
- A nonrefundable 2,000 USD tuition deposit. Your deposit can be made by credit card, check, e-check, or money order. Please plan accordingly so you do not miss the deposit deadline
- Hard copies of your official transcripts and translations (if not in English) from each college or university you attended
- Official test scores sent directly from the testing agency unless you were granted a test score waiver
